A CPAP is medical equipment, not a gadget you hope the airline will tolerate. U.S. screening rules, battery watt-hour limits, and the Air Carrier Access Act decide what happens at the checkpoint and at the gate. The practical work is packing so those rules do not surprise you at 5 a.m.

Should the machine go in the cabin or in checked baggage?

Keep the device with you. TSA allows a CPAP, BiPAP, or APAP in carry-on or checked bags, but a checked bag can be delayed, crushed, or sent to the wrong city, and the night you land is the night you still have obstructive apnea. ResMed and other manufacturers tell patients the same thing for a simpler reason: the blower is easier to replace than a week of untreated sleep.

Under the Air Carrier Access Act, a CPAP is an assistive device. The Department of Transportation’s passenger guidance says assistive devices do not count toward a baggage limit, with an important caveat: if the same bag is stuffed with clothes and souvenirs, the airline can treat it as ordinary luggage. Pack the machine, mask, tubing, power supply, and batteries in a dedicated case. Put shirts somewhere else.

That cabin-versus-checked choice also matters for lithium batteries, which is a separate federal rule, not a TSA preference. Spare lithium-ion packs and power banks are carry-on only. If a gate agent takes your roller bag at the door, pull the spare batteries out and walk them onto the aircraft. FAA PackSafe is explicit on that point because cabin crews can fight a battery fire; the cargo hold cannot in the same way.

If you still use CPAP therapy at home, treat travel as a continuation of that prescription, not a vacation from it. Skipping nights because the bag was inconvenient is the same as skipping nights on Alkyre Run Drive.

What does TSA actually do with a CPAP?

Two TSA pages disagree slightly, and you should plan for the stricter one. The agency’s “What Can I Bring” entry, updated 15 July 2026, says nebulizers, CPAPs, BiPAPs, and APAPs, plus masks and tubing, may stay in the carrying case for X-ray, but officers may require the device to come out of the bag. A separate TSA FAQ says a nebulizer, CPAP, BiPAP, or APAP must be removed from its case and X-rayed, while facemasks and tubing may stay in the case, and you may put the machine in a clear plastic bag for the belt.

In practice: empty the humidifier chamber at home, bring a gallon-size clear bag, and be ready to lift the blower out of the case. TSA PreCheck screening is more likely to leave the device in the bag. The officer still has the last word. That is printed at the bottom of the What Can I Bring page, not a rumor from a travel blog.

Explosives-trace swabbing of the housing is ordinary. It is not an accusation. Wipe the machine when you get to the gate if you care about the film they leave on the plastic.

Water is the part people get wrong. The What Can I Bring page exempts liquids associated with nebulizers from the 3-1-1 rule in reasonable quantities. It does not give distilled water for a humidifier the same sentence. Medically necessary liquids can be declared in larger amounts, but a bottle of distilled water is easier to buy after you land than to argue about at the checkpoint. Empty the chamber before you leave Westerville. Tap water in a humidifier leaves mineral scale; pharmacies at the destination usually stock distilled water if you ask.

Which batteries are legal in the cabin?

FAA PackSafe, last updated 11 August 2026, is the document to read, not a retailer “FAA approved” sticker. Spare lithium-ion batteries, including power banks, must travel in carry-on. Terminals need to be protected from short circuit: original packaging, tape on the contacts, or a pouch that keeps metal off the terminals.

Size limits for passenger lithium-ion batteries:

More than 100 Wh, up to 160 Wh Airline approval. Limit of two spares per person.
Over 160 Wh Not permitted on passenger aircraft.

Watt-hours are usually printed on the pack. If you only have volts and amp-hours, Wh equals volts times amp-hours. A battery installed inside a device is a portable electronic device, not a “spare”; spares are the ones sitting loose in the bag. Damaged or recalled packs that can spark or overheat are not supposed to fly at all unless the unsafe battery has been removed.

Individual airlines can be stricter than PackSafe, including how many power banks they allow. Check the carrier’s hazardous-materials page, not a screenshot from 2019.

Can you run CPAP in the seat?

Yes, with conditions that belong to the airline and to DOT, not to TSA. DOT lists CPAP among assistive devices and says devices used onboard may need a manufacturer label showing they meet FAA requirements. To use the machine in flight, an airline may require up to 48 hours’ notice, a medical certificate if they reasonably doubt you can complete the flight without extraordinary medical help, enough charged batteries for at least 150 percent of the flight duration, and check-in an hour before the usual time. Batteries must be non-spillable, marked as such or enclosed in a case.

Those 150 percent and 48-hour items are permissions the airline may impose, not a ritual every domestic hop requires. A two-hour Columbus-to-Chicago day flight is a different conversation from an overnight to Europe. Call the carrier’s disability desk. Ask whether cabin power is offered, whether they want the device on battery only, and whether a humidifier is allowed (many crews prefer the chamber empty in the air because spills and turbulence mix badly).

Seat outlets fail. If you need the machine for the night portion of a long-haul, the battery plan is the plan. Do not discover at 35,000 feet that the in-seat USB port will not run a 60-watt brick.

What about hotels, voltage, and extension cords?

Most current travel and full-size PAP machines accept 100–240 V, 50/60 Hz. Confirm that on the power-supply label before you assume a plug adapter is enough. An adapter changes the prongs. A transformer changes voltage. Using the wrong one is how people cook a $900 brick in a Lisbon outlet.

U.S. hotels are simpler: grounded outlets, sometimes behind the nightstand. Bring a short, grounded extension cord so you are not sleeping with the hose stretched across a lamp. Cruise cabins and older inns can have few outlets; a power strip that is not a surge-protector brick with a huge lithium pack is the boring solution.

If the trip is camping, a cabin without reliable mains, or a long layover, that is when the watt-hour table above stops being theoretical. Size the pack from your machine’s typical draw with the humidifier off, because humidifiers consume a large share of the wattage. Manufacturer run-time charts assume a specific pressure and a specific humidifier setting. Your download from home is a better estimate than the box.

People who already struggle with leak or pressure at home will struggle more in a dry hotel. That is a mask and humidity problem, not a reason to abandon therapy for a week. If travel is the season you stop using the device altogether, say so at the next visit. There is a separate conversation about CPAP alternatives when the machine is not going to be on your face in the places you actually sleep.

What belongs in the bag the night before?

  • Blower, mask, extra cushion, tubing, filters, power supply, and the dedicated case.
  • Clear plastic bag for the X-ray belt.
  • Copy of the prescription or a sleep-physician letter if you want less discussion at a foreign desk. TSA does not require a letter for screening. Some airlines and some countries do for onboard use.
  • Manufacturer label or IFU page that shows FAA-compliant use, if you intend to run the device in the seat.
  • Spare lithium pack under the correct Wh limit, terminals covered, in the same carry-on.
  • Empty humidifier chamber. Distilled water bought after you land, or a small declared medically necessary bottle if you have a reason not to wait.
  • A note of the pressure or APAP range you actually use, in case a durable-medical supplier at the destination has to swap a broken unit.

Does any of this change on an international ticket?

Security agencies outside the United States are not TSA. Some will never have seen your model. Keep the device accessible, keep the letter, and assume you will unpack it. Lithium watt-hour limits are widely aligned with ICAO figures that look like PackSafe, but a given carrier can still refuse a 150 Wh brick that another carrier approved. Get the approval in writing before you leave Ohio.

Voltage, plug shape, and distilled-water supply are the three things that actually strand people. Distilled water is a pharmacy item in much of Europe. In some countries you will be offered “deionized” or “for irons.” If you cannot confirm it is distilled or demineralized water suitable for a humidifier, skip humidity for a night or two rather than filling the chamber with whatever is in the minibar.

None of this is a reason to leave the machine at home. Untreated moderate or severe obstructive apnea does not take vacation days. The paperwork is annoying. The physiology is not optional.
