New-patient paperwork can be completed before your appointment. Ring the office and we will get the right forms to you.

The practice’s forms are hosted with a third-party form provider, and we are not linking to them from this site until it is confirmed that the account is on a HIPAA-enabled plan with a signed business associate agreement. Until then the office will post or email them to you.

- New patient information. History, current medicines, insurance details.
- Release of information. Authorises another practice to send us your records, or us to send yours on.
- Provider referral. For your doctor’s office to complete and fax to (614) 898-9350.
What to bring
- Your insurance card and a photo ID.
- A list of every medicine you take, including anything over the counter.
- Any previous sleep study reports, if you have them.
- Your CPAP machine and mask, if you have one and it is not working.
